Tony Barton Soccer Academy – Permission & Media Release Agreement
Parent/Guardian Permission & Liability Agreement
By registering my child for training with Tony Barton Soccer Academy, I acknowledge and agree to the following:

1. Assumption of Risk
I understand that participation in soccer training involves physical activity and carries a risk of injury.
I agree that my child is voluntarily participating and is physically able to do so.

2. Liability Release
I release and hold harmless Tony Barton, Tony Barton Soccer Academy, its coaches, staff, volunteers, and affiliates from any liability, claims, or demands arising from my child’s participation in training sessions, games, clinics, camps, or academy events.

This includes, but is not limited to:

Accidental injury

Property loss

Medical expenses

Any risks associated with physical activity

I understand that reasonable safety measures are taken at all times.

3. Medical Authorization
In the event of an injury or emergency, I authorize the academy staff to seek appropriate medical care for my child.
I understand that I am responsible for any medical costs incurred.

⭐ Media & Footage Release
4. Permission to Use Photos & Video
I grant Tony Barton Soccer Academy permission to record, photograph, and capture video of my child during training sessions, games, clinics, and academy events.

I authorize the academy to use this media for:

Website content

Social media posts

Training footage

Marketing materials

Player development review

Highlight videos

5. No Compensation
I understand that all photos and videos are the property of Tony Barton Soccer Academy, and I will not receive compensation for their use.

6. Opt‑Out Option
If I prefer my child not to appear in photos or videos, I will notify the academy in writing.
I understand that the academy will make reasonable efforts to honor this request.
