Tony Barton Soccer Academy - Permission & Media Release Agreement

Parent/Guardian Permission & Liability Agreement
By registering my child for a clinic with Tony Barton Soccer Academy, I acknowledge and agree to the following:

1. Assumption of Risk
I understand that participation in soccer activities involves physical activity and carries a risk of injury. I agree that my child is voluntarily participating and is physically able to do so.

2. Liability Release
I release and hold harmless Tony Barton, Tony Barton Soccer Academy, its coaches, staff, volunteers, and affiliates from liability, claims, or demands arising from my child's participation in clinics or academy events, including accidental injury, property loss, medical expenses, and risks associated with physical activity. I understand that reasonable safety measures are taken at all times.

3. Medical Authorization
In the event of an injury or emergency, I authorize academy staff to seek appropriate medical care for my child. I understand that I am responsible for any medical costs incurred.

4. Permission to Use Photos & Video
I grant Tony Barton Soccer Academy permission to photograph or record my child during clinics and academy events and to use that media for website content, social media, training footage, marketing materials, player development review, and highlight videos.

5. No Compensation
I understand that I will not receive compensation for approved use of photos or video.

6. Opt-Out Option
If I prefer my child not to appear in photos or videos, I will notify the academy in writing and understand reasonable efforts will be made to honor that request.

7. Agreement
By submitting this form, I confirm that I am the parent or legal guardian of the participating child, I have read and understand this agreement, and I agree to the terms above.
